Primary care is the foundation of good health, and for too many people in our region, it’s been out of reach. We provide comprehensive primary care for adults, including annual wellness exams, sick visits, lab work and diagnostics, medication management, and health education.

Whether you’re coming in for a persistent cough or haven’t seen a doctor in years, we meet you where you are. No judgment, no barriers — just care.

What to expect at a primary care visit:

Your first visit includes a thorough health history, physical exam, and conversation about your health goals. We take our time — you’re not a number here. Follow-up visits are scheduled based on your needs, and we’ll coordinate any referrals or additional testing you might need.

Chronic Disease Management

Chronic conditions require consistent, coordinated care. We work with you to build a plan that fits your life.

Diabetes (Type 1 & Type 2)

Blood sugar monitoring, A1C tracking, medication management, nutritional counseling, and complication screening.

Hypertension

Blood pressure monitoring, medication optimization, lifestyle modification support, and cardiovascular risk assessment.

Heart Disease

Ongoing monitoring, medication management, coordination with cardiology specialists, and lifestyle guidance.

COPD & Respiratory Conditions

Breathing assessments, inhaler management, exacerbation prevention, and smoking cessation support.

Thyroid Disorders

Thyroid function testing, medication titration, and ongoing monitoring for hypo- and hyperthyroidism.

Other Chronic Conditions

Behavioral Health Services

Mental health is health. In rural Appalachia, where stigma and access barriers make it especially hard to get help, we integrate behavioral health into our primary care practice.

We provide depression and anxiety screening as part of routine visits, medication management for common mental health conditions, crisis assessment and safety planning, and referrals to counseling and specialized mental health services in the region.

Preventive Care & Screenings

The best healthcare catches problems before they start. We offer preventive screenings and wellness services to help you stay ahead of your health, including cancer screenings appropriate for your age and risk factors, immunizations and vaccinations, wellness exams, health education and lifestyle counseling, and blood work and routine diagnostics.
